New User Account
In the Move dialog under Current Issue Type, select New User Account then hit Next >>
If you see the option to populate the Customer Request Type, select New User Account from the drop down and hit Next >>
Then click Move to move the ticket
If you didn't update the Request type in the earlier step, on the right hand side of the screen, select New User Account from the drop down.
Hit "Edit" and update the relevant fields for this Request Type
There are some extra fields needed for this request type. Therefore, towards the bottom of the page, under Forms, click "Add Form"
Under "Choose a form to add to this issue" select "ITSD Service Request New User Account Screen"
You will see a preview of the form, then click "Add" to add it to the ticket.
